PHOENIX, March 26, 2019 /PRNewswire-PRWeb/ -- Former
patients of two Arizona hospitals
that ceased operations in June 2018
have until June 23, 2019 to request
and obtain copies of their medical records.
Gilbert Hospital, 5656 S. Power Rd., Gilbert, Ariz., and Florence Hospital at
Anthem, 4545 N. Hunt Hwy., Florence,
Ariz., closed last summer after encountering prolonged
financial hardship. A receiver was appointed over the hospitals on
June 6, 2018 to oversee the winding
down of the hospital. Under terms of a court-approved plan related
to the hospitals' medical records, former patients and appropriate
parties have a limited 90 day period until June 23 2019 to request copies of their medical
records. After this date, all medical records will be permanently
destroyed for security measures.
To obtain copies of medical records, former patients and other
appropriate parties must first fill out a medical record request
form for either Gilbert Hospital or Florence Hospital at Anthem,
and send it to nvhrecords (at) resolutecommercial (dot) com. Copies
of the record request forms may also be obtained by either emailing
nvhrecords (at) resolutecommercial (dot) com; calling 480-947-3173;
or mailing Resolute Commercial, the Receiver of Gilbert and Florence Hospital, at 7201 E.
Camelback, Ste. 250, Scottsdale.
After processing and notifying the applicant, requested medical
records will be available for pickup in person at the Resolute
Commercial offices at 7201 E. Camelback, Ste. 250, Scottsdale unless other arrangements are made
with the Receiver's office. Parties requesting a copy of medical
records will need to provide documentation to validate identity ,
such as a photo ID, a power of attorney, or other similar
information over email or during pickup from the Resolute
Commercial Offices.
