05/5/2020 15:35 | This is 'QI' Published yesterday Should we stimulate or suppress immune responses in COVID-19? Cytokine and anti-cytokine interventions 5. Conclusion The rapid diffusion of COVID-19 drives an urgent search for effective treatments, mainly for its severe forms. The disease is polyphasic in nature, with secondary cytokine storm and ARDS resulting in poor outcomes, plus overwhelmed intensive care units and hospitals. In the very early stages of the disease, treatment should focus on reducing viral load either by specific antivirals or by stimulating type-I IFN. Later on, in selected patients, therapies targeting proinflammatory cytokines could suppress hyperinflammation, while some cytokines (IL-7) could theoretically trigger immune restoration. In clinical practice, unfortunately, patients are unlikely to be detected early enough (i.e. before/at symptom onset) to benefit from antiviral strategies. Therefore, factors to predict progression toward severe forms of the disease are, at present, the most urgently needed and awaited determinants. A highly-structured approach, which includes immune monitoring, would thus be of utmost importance. | ![]() waterloo01 | |
