But what we have are "Non Events" making headlines to the detriment of common sense. Could you imagine for just one second if every near miss incident was reported in the news? Today on the A32 there were 27 near misses from people crossing the road. On the M25 there were 1287 reported near misses today from people having to jump on their brakes hard. Its H7S gone mad. I can not even bring myself to say "If XYZ had happened it could have been disastrous" because "It DIDN'T Happen". Safety systems are in place... We know that. Human actions are not infallible, we know that. Machinery can suffer catastrophic failure, we know that. Unplanned events can happen, we know that. If a valve is erroneously left open as a result of human error, or a valve has blown out, it's a safety issue with potentially ruinous consequences.
