But that assumes all orders have to be filled. I could put in an order for a million shares in distil at 3p but it won't be filled at the current price of 3.3p.

Errrr, no. A Market Makers job is to make the market work, not to dictate what the stock is worth.Sharp movements are most likely big orders that need to be filled. For example, a fund makes a deal with an MM, via their broker, for 10m shares at a maximum of 5p per share. The stock price prior to the big order is 4p. So the MM immediately moves the price up to 4.5p, hitting limit sells and encouraging short term traders to offload the stock. The MM can walk the price up and at the same time limit how much they sell to other buyers (often seen in BB posts such as on DIS, where people say "only being offered 2000 shares", or "can't get a price for any amount if stock"). Later on all the delayed trades appear, blocks that are subsequently put together to fill the big buyers order.

It's a cart before horse thing. Surely the MMs will take stock onto their books at the price they judge buyers will buy at. They are market makers. There's little point them taking loads of stock at 4p if buyers are sniffing around at 3.3p for example. Frequently a stock will change price sharply with no trading at all.

You're missing out that the MM's are the buyers and sellers of all the stock, and they must hold a minimum inventory of the stock (to maintain a liquid and orderly market for the stock) in order to be an MM for that stock.The price fluctuates with the increase and decrease in the size of the MM's inventory, that is the core of supply and demand.If an MM can't match a buyer and seller they still have to give a price to a buyer or seller when a quote is requested, and they still have to make the deal if they are offering one. Say a lot of sells go into the MM's inventory, the MM is not seeing enough buyers to pass the stock to, so they drop the price to 1) encourage buyers and 2) to discourage sellers unless said sellers are willing to accept a lower price. And so on.
