( 1)	The price reported in Column 4 is a weighted average price. These shares were sold in multiple transactions at prices ranging from $15.27 to $15.93, inclusive. The reporting person undertakes to provide Five9, Inc., any security holder of Five9, Inc., or the staff of the Securities and Exchange Commission, upon request, full information regarding the number of shares sold at each separate price within the ranges set forth in this footnote.
( 2)	These shares are held by Sapphire Ventures Fund I, LP ("Sapphire"). Sapphire Ventures (GPE) I, L.L.C. is the general partner of Sapphire. Mr. Das is a managing member of Sapphire Ventures (GPE) I, LLC. Mr. Das disclaims beneficial ownership of all shares held by Sapphire except to the extent of his pecuniary interest therein.
( 3)	Under Sapphire's partnership agreement, Mr. Das is deemed to hold the common stock for the benefit of Sapphire. Mr. Das disclaims beneficial ownership of these shares except to the extent of his pecuniary interest therein.
