DUBLIN, Ohio, Oct. 7, 2015 /PRNewswire/ -- The Cardinal Health Foundation awarded 29 grants totaling $924,551 to help non-profit health care institutions across the country improve the effectiveness, efficiency and excellence of patient care.

The E3 Patient Safety Grant Program supports three types of projects this year:

  1. Projects that improve medication safety as patients transition from the hospital to home.
  2. Projects that use technology, including the use of smartphones, apps, tablets, remote monitoring and telehealth through consumer devices, to increase access to care and engage patients.
  3. Projects that help spread previously successful work.

The Cardinal Health E3 Grant Program awarded funding of up to $45,000 per grant to health care providers in 18 states. Since the inception of the E3 Grant Program in 2008, the Cardinal Health Foundation has awarded just under $8 million in funding to 284 hospitals, health systems or other health-related organizations across the country.
