17/4/2009 10:03 | Elmfield, I have filled in the form for my brokers (my shares were held in ISAs) - there was no need for a photo, just details of passport number, date of issue etc. There were two forms to fill out, and for people not used to this kind of thing I imagine they may have some difficulty completing them as they got a little technical at times! | rivaldo | |
17/4/2009 09:59 | elmfield,when i saw a declaration form online the other day,did not see anything about a photograph,it did require passport details and signature etc. just trust my broker has it right,it maybe different brokers have different ideas about it.but dmatek is not the first israeli company on the lse to be taken over, so you would think the ins and outs of it would have been ironed out by now. | funchalman | |
17/4/2009 09:54 | Elmfield You are right - it is a flaming mess. I'm with both Selftrade and idealing. Both have confimed that there is a form to be filled in by individual shareholders, which contradicts the report in post 1135. Both say they are sending it out today or Monday. You will certainly need a signature on the form, but nobody has mentioned passport photos. The form has to signed an returned to them as an original - ie not faxed or scanned then sent electronically The good news (if its true) is that idealing says that they can find nothing in the documents they have to support April 22 as being a drop-dead date. | 3offthet | |

16/4/2009 10:35 | well i spoke just before to my broker and they have confirmed that they have filled in the israeli tax form,and have already sent it off.said my name does not appear on the holding,so they have filled it in.so feel more relaxed about that,cant do anymore.asked for a date for receipt of money,they couldnt give one. | funchalman | |
15/4/2009 19:41 | ref the israeli tax declaration,i have had a look at such a form on the internet,and yes there are questions on it that the broker could not answer. my broker said they was in the process of completing the form,whatever that means,they didnot say they was going to send one to me.one has to presume they know what they are doing,but i will hang on to their email,just in case.may even give them a call tomorrow,as 25% is serious money. | funchalman | |
15/4/2009 17:02 | funchalman - I certainly agree that a nominee broker should be held liable if they screw things up. But in this case I'm not sure how they could properly 'handle all the paperwork' for nominee holders (like me). The Israeli authorities ask detailed questions about an investor's status/relationships GerdMuller - glad for your sake that you have it in writing from Barclays, because Barclays' 8 April letter to me was very clear that I wouldn't be paid in full unless they did get the Israeli forms back by 22 April. No doubt I would still be paid minus the 25% withholding tax - but, FWIW, there is nothing in the letter to indicate that payment can be expected this Friday. On the contrary, the letter just says "we will endeavour to update your account within five business days of receiving the proceeds" - with no indication of when that might be. | mrwoo | |
15/4/2009 10:58 | I too am with selftrade. Following my phone call today, they informed me that the Israeli tax form needs to be filled out by me and that they would be sending them out shortly. I pointed out the completion date of 22nd April and they said that they were aware of it. We will have to wait and see. | geoffro | |
15/4/2009 10:13 | funchalman, I think you are right. I am not returning to the UK until next week and have been away quite a while so I dont know what I have been sent. But when I contacted Barclays last week they said I would be paid by this Friday. I asked if I needed to take any further action and they said no in their email. If I have not been paid by friday I will contact them. My spread bet has not yet been settled but they also said I need take no further action. | gerdmuller | |
14/4/2009 19:08 | all my shares are held in nominee accounts and the nominee broker should have taken care of all the paperwork,but thought i would check on that tax form.the reply to my email said the declaration was in the process of being completed so happy with that.would have had a coronary if any tax had been deducted. think it will be next week before i see any money,cant see it being this week. | funchalman | |

14/4/2009 12:54 | Yes, I received the form (in two parts, both have to be completed) from Barclays last week. The covering letter from Barclays explicitly said "if we do not receive the completed forms by 22 April you will not receive your full entitlement". | mrwoo | |
